PHA Guidelines:
Preliminary Hazard Analysis is an important tool that is used to identify potential system safety risks, and to allow development of safety mitigations into the design.
A good PHA at a minimum includes: Hazard ID, Vehicle Level Hazard, System Level Hazard, Worst-Case Mishap, Comments for selecting each Severity, Exposure, and Controllability Levels, ASIL rating, Safety Goals, and Safety Requirements.
Traceability Matrix Guidelines:
Traceability matrix is used to show a clear connection between each system requirement/design and the corresponding safety goal.
A good design should not have any safety requirement/design that are not linked to one or more safety goal. For a traceability matrix to be useful, it must include hazard IDs, requirement IDs, design IDs, etc. that are consistent with all other documents.
